Healthcare Equality Index: Participation Request

If your healthcare organization would like to participate in the Healthcare Equality Index, just complete the form below. When the HEI reopens in Summer 2013, the representative named will receive a unique login that your organization can use to access the HEI 2014 without any obligation to participate.

Please note that the HEI is designed for healthcare facilities with ten or more employees, not smaller or individual practices. Small practices and solo practitioners are encouraged to explore the GLMA Provider Directory.
