Healthcare Equality Index: Login Request
If your healthcare organization would like to view or participate in the Healthcare Equality Index 2013, just complete the form below. The representative named will promptly receive a unique login which your organization can use to access the HEI 2013 without any obligation to participate. The HEI 2013 closes on March 1, 2013, so be sure to request your login soon.
Please note that the HEI is designed for healthcare facilities with ten or more employees, not smaller or individual practices. Small practices and solo practitioners are encouraged to explore the GLMA Provider Directory.
