All licensed, accredited agencies, both public and private, and adoption exchanges are eligible to participate in All Children - All Families. Please complete the form below to request your agency's log-in information for the All Children – All Families Agency Self – Assessment.
Note: We are currently updating the Agency Self-Assessment. As soon as these updates are finalized (August 2013) all new participation requests will be processed and your agency will receive its unique login information for the Assessment.
